INDICATIONS FOR ECT
ECT INDICATIONS: 1. ACUTE INDICATIONS: •Acute Severe depression with suicidal ideation with or without psychosis •Acute mania with or without psychosis •Catatonia PRIMARY USE OF ECT: Used prior to psychotropic trial when •Rapid, definitive response is needed • Risk of medical and endocrinological side effects medications are greater than the risk from ECT •Poor response to medicines in previous episodes •History of good ECT response SECONDARY USE OF ECT: •Treatment resistance •Intolerance or adverse effects to psychotropic mediations •Deterioration of medical and psychiatric condition PSYCHIATRIC INDICATIONS: Bipolar affective disorder: •Depressed •Mixed •Manic Schizophrenia: -Catatonic type -Abrupt psychotic onset -History of favorable response -Schizoaffective disorders Medical disorders: •Parkinson’s disease •NMS (neuroleptic malignant syndrome) •Intractable seizure disorder Geriatric Population: • Patients Sensitive to Side effects of psychotropic medications •Very responsive to ECT Pregnancy: •Severe depression with or without suicidal ideation with or without psychosis •Risk of exposure to psychiatric medications with potential for congenital malformations is higher and ECT is a safe alternative treatment |
